The card information gained from the effect remains as-is. For example, if EX1-005 [Tyrannomon]'s original card information is changed, its original color will become white, and EX1-005 [Tyrannomon]'s effect will also treat it as green, making it a 2-color white and green Digimon.

It remains changed to a white Digimon with an original name of [Sukamon] and 3000 DP, but any card information that it didn't originally have can't be changed. If it becomes a Tamer or Digi-Egg card, or if it becomes a card that didn't originally have DP, then it will remain without DP.
